Follow these steps to change the nominations of your policy.
Choose the correct Service Request Form and complete all the fields accurately
Provide all required official signatures
Provide the necessary identity documents
Submit the completed form(s) together with supporting documents via your financial consultant or email to Customer Service Centre together with scanned supporting documents. Alternatively, you may mail it to us, attention to “Customer Service & Operation Department”.
When you, the policyowner, make a revocable nomination, you will continue to retain full rights and ownership over the policy. This means that you can change or revoke a nomination at any time without the consent of the beneficiary or beneficiaries. All rights over the policy will be subjected to the terms and condition of the nomination created.
To nominate immediate family members who have insurable interest like spouse, parents, children, grandparents, the following documents are required.
A completed and signed Change of Nomination/Beneficiary Form
A copy of NRC Card/ Passport as ID proof (mandatory)
A copy of Birth Certificate (Optional)
A copy of Household statement (Optional)
If the designated beneficiary is younger than 18 years old, the policyholder/ proposer is required to nominate the guardian like Life Assured’s parents, grandparents, or immediate family member as an appointee or trustee on the request form.
To nominate other relatives or entities other than immediate family members, the case would need to be reviewed.
A completed and signed Change of Nomination/ Beneficiary Form
A copy of Household statement (Optional)
A written declaration explaining the reason for nominating the stated beneficiary(ies) who does not have any insurable interest.
